Safety occurrence report can be submitted anonymously. If you do provide your name and contact details they will be used only by Safety Manager and only for the purpose of providing additional information on reported occurrence. All persons who report a security event are guaranteed identity protection in accordance with GDPR requirements.
Remark. This report is concerned safety occurrences other than incidents, serious incidents and accidents according to L-13 regulation which must be submitted to Air Accidents Investigation Institute and Czech CAA on the form Notification of accident and incident.
